Intra- and interobserver concordance of a new classification system for myopic maculopathy

Abstract Background Myopic maculopathy (MM) is one of the major causes visual impairment and irreversible blindness in eyes with pathologic myopia (PM). However, classification each type lesion associated MM has not been determined. Recently, a new system, known as ATN grading was proposed; it based on fundus photographs optical coherence tomography (OCT) images includes three variable components: atrophy (A), traction (T), neovascularization (N). This study aimed to perform an independent evaluation interobserver intraobserver agreement for recently developed system MM. Methods retrospective study. Fundus OCT 125 patients (226 eyes) various MMs were evaluated classified using by four blinded evaluators (2 attending ophthalmologists 2 ophthalmic residents). All cases randomly re-evaluated same observers after interval 6 weeks. The kappa coefficient (?) 95% confidence (CI) used determine agreement. Results substantial when considering (A, T, N). weighted Fleiss ? values N) 0.651 (95% CI: 0.602–0.700), 0.734 0.689–0.779), 0.702 0.649–0.755), respectively. subtypes good or excellent, except stages A1, A2, N1, which value less than 0.6, moderate types > 0.8. No significant differences observed between residents reliability reproducibility. Conclusions allows adequate among different qualifications observer separate occasions. Future prospective studies should further evaluate whether this can be better implemented clinical decision-making disease progression assessments.
